Accident Summary:
AT 1015 HOURS ON 29 JAN 70, CW2
ROSE AND WO1 SMITH TOOK OFF FROM BINH LONG IN OH-6A #66-12965 ENROUTE TO
DANG TAM AAF. CW2 ROSE WAS ACTING AS INSTRUCTOR PILOT AND GIVING WO1 SMITH
HIS 90 DAY STANDARDIZATION RIDE. UPON ARRIVING AT DANG TAM, AFTER BOTH
MR ROSE AND WO1 SMITH DID SEVERAL NORMAL TAKEOFFS AND LANDINGS, SEVERAL
MAXIMUM PERFORMANCE TAKEOFFS AND STEEP APPROACHES, CW2 ROSE TOOK THE CONTROLS
AND DEMONSTRATED TWO BASIC AUTOROTATIONS. FOLLOWING THIS, WO1 SMITH MADE
A NORMAL TAKEOFF, NORMAL LANDING, ANOTHER NORMAL TAKEOFF, AND ENTERED AUTOROTATION
AT 500 FT AND 100 KTS ON FINAL. HE STATED THAT HIS ROTOR WAS IN THE HIGH
GREEN AND HE HAD 61% N-1. AT 50 FT HE DECELERATED AND AT 10 TO 15 FT BEGAN
TO LEVEL THE AIRCRAFT AND PULL INITIAL PITCH. WHEN PASSING THROUGH 3 FT
HE CONTINUED TO PUL COLLECTIVE PITCH TO CUSHION THE AIRCRAFT ONTO THE GROUND.
BOTH PILOTS STATED THAT IT WAS NOT A HARD LANDING AND THAT THEY CONTACTED
THE GROUND WITH ONLY A SLIGHTLY TAILLOW ATTITUDE. THEY HEARD A "BANG" DURING
THE GROUND RUN AND WHEN THE AIRCRAFT STOPPED, WO1 SMITH GOT OUT AND FOUND
THE TAILBOOM SEVERED. SQUADRON WAS CONTACTED AND SENT AN AIRCRAFT TO DANG
TAM. CW2 SUARES TOOK PICTURES OF THE AIRCRAFT AND EXAMINED IT AT THE SCENE
OF THE ACCIDENT. CW2 ROSE STATED THAT HE HAD HIS HAND BEHIND THE CYCLIC
DURING TOUCH-DOWN BUT DID NOT BELIEVE IT NECESSARY TO MAKE A CORRECTION.
THE AIRCRAFT HAD A GROUND RUN OF APPROX 12 FT. THE RUNWAY AT DANG TAM IS
IN EXCELLENT CONDITION (AS CAN BE SEEN IN THE PHOTOGRAPHS). THE AIRCRAFT
WAS LANDING TO THE SOUTH WITH THE WINDS OUT OF THE SOUTH-SOUTHEAST.\\

Accident Summary:
AT 0800 HOURS, 18 JUNE 1970,
CAPTAIN RHETT HAD COMPLETED HIS PRE-FLIGHT INSPECTION AND DEPARTED VINH
LONG AIRFIELD ENROUTE TO DONG TAM. CAPTAIN RHETT WAS INSTRUCTED TO LAND
AT DONG TAM FOR A BRIEFING PRIOR TO BEGINNING HIS TACTICAL MISSION. HE
BEGAN HIS APPROACH TO DONG TAM AIR STRIP AFTER COMPLETING G A PRE-LANDING
COCKPIT CHECK WHICH INDICATED ALL SYSTEMS FUNCTIONING PROPERLY. AT APPROXIMATELY
30 FEET FROM TOUCHDOWN, THE PILOT BEGAN A GRADUAL POWER APPLICATION IN
PREPARATION FOR THE TERMINATION OF HIS APPROACH. AT THIS TIME THE AIRSPEED
INDICATOR SHOWED APPROXIMATELY 10 TO 15 KNOTS. WHEN HE BEGAN THE POWER
APPLICATION, HIS ROTOR RPM BEGAN TO DETERIORATE. HE REDUCED COLLECTIVE
PITCH IN AN EFFORT TO REGAIN HIS ROTOR RPM WHICH HAD DROPPED TO 450. THE
RPM CONTINUED TO DECREASE TO APPROXIMATELU 415. CAPTAIN RHETT MAINTAINED
THE COLLECTIVE PITCH AT ITS LAST SETTING UNTIL JUST PRIOR TO TOUCHDOWN.
HE THEN ATTEMPTED TO CUSHION HIS LANDING BY APPLYING THE REMAINING COLLECTIVE
PITCH. THE AVAILABLE COLLECTIVE PITCH WAS NOT SUFFICIENT, AND A HARD LANDING
RESULTED.\\
