Accident Summary:

THE OH-6A, ^SN 67-16187^, REQUIRED A TEST FLIGHT AFTER EXTENSIVE MAINTENANCE ON THE ENGINE FUEL CONTROL AND REPLACEMENT OF THE GOVERNOR. THE TECHNICAL INSPECTOR HAD COMPLETED HIS INSPECTION AND SIGNED OFF THE 2408-13. THE TEST PILOT HAD FLOWN 151 HOURS IN 30 CONSECUTIVE DAYS AND WAS ON HIS DAY OFF. HE WAS AWAKENED TO PERFORM THE TEST FLIGHT. UPON ARRIVING AT THE AIRCRAFT, THE TEST PILOT COMPLETED HIS PREFLIGHT INSPECTION AND PREPARED TO TEST FLY THE AIRCRAFT. AFTER A NORMAL START AND RUN-UP, THE TOWER CLEARED THE PILOT TO HOVER TO "B" ROW FOR DEPARTURE. THE ROUTE FROM THE TROOP RUN-UP AREA TO "B" ROW PROVIDED THE PILOT WAS WITH A RIGHT QUARTERING TAILWIND. IT SHOULD BE NOTED THAT THE PILOT HAD NO CHOICE BUT TO BE HOVERING DOWNWIND AT THE TIME OF THE ACCIDENT. AS THE PILOT HOVERED TO "B" ROW, RPM BEGAN TO BLEED OFF AND THE PILOT ENTERED AUTOROTATION. THE AIRCRAFT HIT HARD CAUSING THE MAIN ROTOR BLADES TO FLEX AND SEVER THE TAIL BOOM.\\
